The most important and glaring issue is the fact that your actions don't matter or affect anything in this game. Every NPC states just how important your character is but, when you actually do something important, (like say, murdering an entire village or becoming the king of an ancient race) nothing happens, everything stays the same.
In a particular quest a character ends up saying "this whole land is ours now" but it's just exposition, you end up being the ruler of nothing. It's all a big lie.
